New Patient Appointments:

Please arrive 30 minutes prior to your scheduled appointment time.

We will need the following the day of your appointment:

Drivers Licence or Photo ID & Insurance Cards.

Please be prepared to pay your Co-payment and or Deductible.

* Medicare annual deductible for 2017 is $183.00 *

When scheduling your new patient appointment, we will ask you for an email address. This will register you for our patient portal.

We ask you log on to our portal and complete our New Patient History forms as well as other forms required prior to seeing the physician.

This will expedite your check in process, you will also have the capability to cancel and reschedule your appointments through our portal.
